Fees and Services
Rates from July 2025. All fees are inclusive of employer's NHS Pension contribution of 14.38% and reflect the recent 4% DDRB funding uplift.
All consultations (with patient consent) use Tortus AI - NHS compliant AI assisted scribing tools. This improves the quality, accuracy and detail of clinical notes. All dictations and other letters are generated using Dragon Medical voice recognition, significantly reducing secretarial workload.
Full Day: £840
Includes 25 patient contacts (as per BMA safeworking model) or equivalent, up to 2 visits (upto 45 min each), any emergency visits / appointments and as much administration/ paperwork / letters / results / prescription work as time allows. Alternatively following the practice’s standard GP working patterns or working as duty doctor for a full day.
Half Day: £425
Includes 13 patients (as per BMA safe working model), up to 2 visits (upto 45 min each) and as much administration / paperwork / letters / results / prescription work as remaining time allows. Alternatively following the practice’s standard GP working patterns or working as duty doctor for a full day.
Surgeries
Includes dictation / paperwork / referrals for those patients seen. Face-to-face appointments are 10-15 minutes, telephone consultations 5-10 minutes and any combination is possible.

Home Visits
Allows 45 minutes per visit and includes travel up to 4 miles/visit & all related administration.

Enhanced Access / Extended Hours
Outside core hours: Mon-Fri 0800-1830 (assuming a standalone shift – other rates apply if already on site or blend of in and extended hours)

Booking
Availability
Neil offers live on-line availability through two complementary GP Locum services: GPNetworks and LocumOrganiser.
The GPNetworks diary is freely available but a little limited whilst the LocumOrganiser diary is superior but requires client practices to sign up (but it is free to use).

Notes About Fees
Neil charges all his fees at a fully inclusive rate including NHSPA Pension Employer’s Contributions. Provision of medical services is VAT exempt.
Invoices are issued monthly for settlement within 28 days. Administration fees and interest are applied to late settlements.
Some practices choose to ask doctors to type and send their own letters, rather than dictate them. Neil is happy to do this but additional time is required to do this properly and safely. Wherever possible Neil uses his own Dragon Medical dictation software to work as effciently as possible on behalf of clients.
This service is already included in half and full day bookings but extra time will be charged when surgery-only bookings have been made. Basic form filling is included in surgery-only bookings as is dictation of letters.
Working with Locum Agencies
Neil sometimes works with locum agencies to fill niche skill gaps and hard-to-cover sessions.
Locum agencies are welcome to contact Neil if they have sessions and he has availability but they should ensure they are able to offer competitive rates. Speculative approaches are not welcomed and waste everyone’s time.

Terms and Conditions
Dr Neil Iosson undertakes to provide the following services when contracted to do so:
- Fully qualified General Practitioner, accredited by the GMC.
- Included on South-East Provider list and regularly appraised and up-to-date through continuing education
- Full indemnity through Medical Defence Union
- All basic clinical examination equipment and car (fully insured) for visits
Making a booking is acceptance of the terms and conditions outlined below.
Cancellation: Once a booking has been confirmed by both parties by telephone, email or letter it is binding. In the event of cancellation by the client: cancellation fees of 75% will apply if cancelled within 72 hours, 50% will apply if cancelled within 14 days of the due start date and 25% between 14 and 28 days. Dr Iosson will not cancel any booking except due to exceptional circumstances such as illness, injury etc. He will endeavour to find a replacement locum where possible but cannot guarantee to do so and is not responsible for failure to do so, any difference in a replacement locum’s charging structure and is not liable for any fees or consequential loss.
Confidentiality: Dr Neil Iosson guarantees both patient confidentiality as required by the GMC and all aspects of commercial confidentiality for his employer.
Parking: If a car is required for planned activities and a parking space is not provided, any applicable car parking fees will be charged back to the surgery at cost.
Surgery duration: A surgery is defined as starting from the first appointment time to the end of the last planned appointment. Overruns and time for administration, form filling, dictating letters etc are not charged. Practice managers must decide whether they wish to book additional time for “extras” – this can be negotiated on the day if necessary. For those practices that choose not to provide medical secretaries to type up referral letters and instead ask doctors to do this – Dr Iosson is very happy to do so but any additional time required to do so will be charged at usual rates (unless part of a half or full day surgery booking).
Settlement terms: Invoices will be issued within 28 days, to be settled in full within 28 calendar days. Under the Late Payments of Commercial Debts (Interest) Act an administration fee (£50 for debts up to £999.99; £85 for debts up to £9999.99, £120 for debts up over £10000) will be charged for late payment and interest at 2% per month compounded each month will be charged in accordance with English law until the invoice is settled in full.
Implementation: Fees effective from 1st April 2024. Making a booking for dates after 31 March 2024 is considered acceptance of terms and conditions and fees charged unless different terms have been agreed in writing.
Data Privacy Impact Assessment (DPIA) & AI tools: Consultations are delivered using Tortus AI scribing technology, a regulated Medical Device. This is used after patient consent has been obtained. Tortus AI is approved and validated for use within the NHS for patient consultations. Audio is temporarily recorded on a local computer, uploaded to a secure cloud server for processing and then the AI analysed text is downloaded. The text is reviewed, manually edited for accuracy and copied into the GP clinical system. Audio recordings on the server are immediately destoyed after the consultation analysis and neither the audio recording nor the transcription of the consultation are retained or stored either on the server or the local computer beyond the end of the session. Data generated is not shared, stored or used outside of the immediate consultation. If a patient opts out of AI consultations then the consulation notes will be generated in the conventional way.
Data Processing Agreement: Dr Neil Iosson acting as a contractor for the practice is also acting as a data processor on behalf of the practice. Making a booking for Dr Iosson’s services is also an agreement for Dr Iosson and his employees and service providers to act as data sub-processors for the practice for the relevant purposes to deliver the contracted work. Dr Neil Iosson has data sharing / data processing agreements in place with the following organisations for processing personal identifiable data:
